Odd sign up form

I noticed that when I use the short code[affiliates_registration] on my site, I’m not getting a very pretty table. It looks like this:
FIrst Name __________ Last
Name __________ Username
__________ Email
__________ Website
__________ [Sign Up]

I’ve used __________ to stand in place for a text input box. I see the same behavior in IE11 and Chrome version 35.

    No. Not that I know of. I supposed it might have been a change to the parent theme, but it seems odd that there would have been something there that specific to this form.

    Regardless, I was able to fix it with some new additional css.

    Did something about the form’s tagging change recently?

    My form was fine, now it’s all messed up like the other user described.

      Hi Michael,

      Not really, did you change your theme or maybe you had some customized CSS in the parent theme instead of using a child theme?

    Hi,
    maybe a problem with your theme’s css. Affiliates use a basic html based in class that themes and users can customize.
    Use css for that purpose.
