Nowhere in your documentation does it tell you how to actually refer an affiliate using a form. in other words what does the user enter into the form that will give the credit to the referrer or distinguish (ID) them among 30 of my affiliates?

That’s true, we need to update the documentation.
Please try with a field with id ‘affiliate_id’ so the user can set the affiliate’s id in this field.
Also you can use a field with id ‘affiliate_login’ if the user set the affiliate username.
