Hi Scott,
You can use the shortcode [affiliates_registration terms_post_id="5"/] where 5 is the post id of your terms page. Then, go to Affiliates > Settings, Registration tab and choose status Pending for new affiliate registrations.
FYI new affiliate registration approval feature has been introduced in version 2.16.0 of the plugin in case you need to update your currently installed version.
