We couldn’t able to submit affiliate registration form through standard Affiliate area page nor from gravity form (enabled with affiliates gravity form integration and User registration add on).
Please provide resolution for this.

Thanks for the link Suresh.
I tried the link you posted and indeed gave me a blank screen, but also i tried the affiliates registration form which gave me some errors. The errors have to do though with fields not present in the form ie “ERROR: Please enter a valid CAPTCHA value.” without the form having a CAPTCHA field but also there are errors which are not rendered by the Affiliates plugin’s error handling ie “Sorry, Day Phone is a required field.”
Can you also provide us me access to your Dashboard?
Make sure you have a full backup of your site and then if possible send your admin credentials to support at itthinx dot com and please don’t forget to include a link to this topic in your message.
