I discovered by activating affiliate for formidable form, the form page would go blank.
Everything comes back to normal upon deleting the affiliates in the Settings>Form action.
Any help?

Thanks for mentioning that issue.
What happens if you enable debugging and visit the form while Affiliates Formidable Form is enabled? Is there anything logged?
Also, could you please indicate which version of Formidable Forms plugin do you use?
