Hi Eugen,
I set up everything again and this time I got it working. Key here is to set ‘Rates’ and then go in and make sure each affiliate is set to use WooCommerce Add On plugin and I think one other little detail. If something isn’t set in one place, it doesn’t appear as an option in another.

We have decided to continue to use this plugin and at some point hope to find a solution in getting different affiliates each to receive a different percentage.
