There something broken with the translations. The folder core/languages contains the file affiliates-it_IT.(po | mo) but the text is rendered in English.
As an example the label “first name” in the “Affiliate information” form. The .po file contains the correct translation ( I’ve checked with poedit ), but the label is rendered in English.
Update : We are adding support for registration field label translations with WPML String Translation to our upcoming new releases (this affects all versions of the system).
To make use of this new feature, WPML’s “Multilingual CMS” version is required (you will not be able to have string translations if you only have a license for the “Multilingual Blog” version).
If you do not need to run a multilingual system, then you don’t need the above. You can simply translate the default labels under Affiliates > Settings > Registration if they do not already appear translated.
If you want to run a multilingual system, then you can add the appropriate translations under WPML > String Translation.

Thanks for your patience. I’ve been looking into the issue you describe on a test installation, from what I see this should be fixed from our side and is not related to your site in particular. The translations should be taken from the translation file as you have correctly pointed out, but it doesn’t happen due to an issue with the load order.
What you can do meanwhile is change the actual field labels used under Affiliates > Settings > Registration – you will find the original English labels used there and can simply replace them with their Italian translations.
