Each time an order fails, it is creating a referral. When a customer tries again and is able to complete the transaction, all 2 (and sometimes 3) referrals are being accepted.

I removed the check box in settings to “allow duplicate referrals” and it seems to have fixed it. Thanks!

Hi Phillip,
The referrals are created when the order is created. This does not depend on whether the payment is made or not.
You should have as many orders as referrals.
