In the downloads section http://www.itthinx.com/downloads you will find complete installation instructions in the file Affiliates Enterprise README – please follow these exactly. Also note that you can download your files there again if needed.

previously all integrations within a file, now that has changed, and you can download those separately.
Enterprise Affiliates only need to download and upload to your wordpress installation (as you can see in your screenshot, directly is the plugin’s files).
